Removal of badges for access to the Bahiana Health and clinics

In accordance with the rules of access to the premises of the Bahiana Health and its clinics, we inform you that, in the period from 31/07/17 to 04/08/17, identification badges for access to our facilities will be delivered from 8 am to 16 pm, at the Academic Departments of Academic Units Brotas and Cabula and from 8 am to 18 pm, at the Graduate/Extension/Research Department of the Academic Unit Brotas, as described below:

• Undergraduate students: Academic Departments of Academic Units Brotas and Kabula;

• Graduate and Extension Students and Researchers: Graduate/Extension/Research Department of the Academic Unit Brotas.


To remove the badges, undergraduates, graduate students and researchers must bring two 3x4 photos and access the AVA Platform, to fill out and print the following forms:

Undergraduate
Print two copies of the “Mandatory Internship Commitment Term at the Medical Center, Dental Center and Clinics of Bahiana Health";

Extension
Non-mandatory internship without Notice and Notice: print the form "Accreditation of Access to Bahiana Health" and two copies of the "Internship Commitment Term";

Graduate
Print the form of "Accreditation of Access to Bahiana Health” and two copies of the “Confidentiality Agreement and Rules of Conduct: Bahiana Health";

Research
- No institutional affiliation - print the form "Application” and two copies of the “Confidentiality Agreement and Rules of Conduct: Bahiana Health";
- With institutional link - print two copies of the "Confidentiality Agreement and Rules of Conduct: Bahiana Health".

Scientific initiation / technological initiation students from Bahiana
Print two copies of the “Confidentiality Agreement and Rules of Conduct: Bahiana Health".


We would like to inform you that, as of 07/08/2017, students/researchers will not be allowed to enter the premises of the Bahiana Health and clinics without identification badge.
